Notification of Death (Shibo Todokesho, 1 copy)
Death Certificate (Shibo Shindan-sho, identical to notification of death and completed by doctor)
Inkan (notifier's personal stamp)
National Health Insurance Card (Kokumin Kenko Hoken-sho, for those who are registered)

Registration is mandatory for all those living in Japan, both foreign and Japanese.
Please come in person to register. If you are under the age of 16, or are unable to come to city hall due to severe illness or other legitimate reason, a relative living with you may come apply as a proxy (please submit a medical certificate or doctor's notice as proof).
